

Getting Started with Services at Dimensional Behavior Interventions
At Dimensional Behavior Interventions, we strive to make accessing ABA therapy as simple and straightforward as possible. To begin receiving services, you will need the following documents:
1. Official Diagnosis of Autism Spectrum Disorder (ASD)
In order to receive ABA therapy, an individual must have an official diagnosis of Autism Spectrum Disorder (ASD). This diagnosis is typically made by a licensed medical professional, such as a developmental pediatrician, neurologist, or psychologist.
2. Referral for ABA Therapy
Once an ASD diagnosis has been made, a formal referral for ABA therapy is required. This referral should come from the diagnosing professional or primary care physician. Please ensure the referral is dated within the last year to ensure its validity.
3. Diagnostic Report
Along with the referral, a copy of the diagnostic report must be provided. This report details the individual’s diagnosis, medical history, and any assessments that were performed. It enables our team to better understand the individual’s needs and begin preparing for services.
4. Current Insurance Information
Please provide current copies of all insurance cards, clearly indicating primary and secondary coverage, if applicable.
Steps to Begin Services
Step 1: Request Services
Once you have gathered the documents listed above, please complete the Request Services Form. This short form notifies our team that you are interested in ABA services and provides the basic information needed for us to begin reviewing documentation and verifying insurance.
Step 2: Complete the Intake Packet
After submitting the request form, you may also complete the Intake Packet. This form collects more detailed information about your child, including developmental history, daily routines, communication skills, and current concerns.
Completing the intake packet ahead of time helps our team better understand your child’s needs and allows us to use the initial assessment time more efficiently.
Step 3: Assessment Authorization
Once your documents and request form are received, our team will review the information and submit a request to your insurance provider for authorization to conduct an ABA assessment.
Step 4: Initial Assessment
Once the assessment is approved by insurance, we will contact you to schedule the initial evaluation. During this appointment, our BCBA will conduct an assessment and begin developing a personalized treatment plan based on your child’s needs.
